چکیده

To describe a patient with drug-induced parkinsonism whose baseline dopamine transporter SPECT resulted abnormal, but whose dopaminergic neuronal density was normal. Background: In vivo imaging techniques investigating presynaptic nigrostriatal function using dopamine transporter (DAT) SPECT or 18F-Dopa PET are currently considered to reliably distinguish a degenerative parkinsonism from a iatrogenic disorder.

Differentiating drug-induced parkinsonism from Parkinson's disease: an update on non-motor symptoms and investigations.

Drug-induced parkinsonism is the second most common cause of parkinsonism after Parkinson's disease and their distinction has crucial implications in terms of management and prognosis. However, differentiating between these conditions can be challenging on a clinical ground, especially in the early stages. We therefore performed a review to ascertain whether assessment of non-motor symptoms, or...

The role of functional dopamine-transporter SPECT imaging in parkinsonian syndromes, part 2.

SUMMARY The functional imaging technique most widely used in European clinics to differentiate a true parkinsonian syndrome from vascular parkinsonism, drug-induced changes, or essential tremor is dopamine-transporter SPECT. This technique commonly reports dopamine-transporter function, with decreasing striatal uptake demonstrating increasingly severe disease. The strength of dopamine-transport...

Teaching NeuroImages: Drug-induced parkinsonism with asymmetrical putaminal DaT binding.

A 64-year-old patient developed shuffling gait and postural instability over 8 months, in parallel to risperidone intake. Examination showed symmetric parkinsonian syndrome and mild left hemiparesis with spasticity, related to right middle cerebral artery stroke 10 years before. Within 1 month of risperdone withdrawal, parkinsonism disappeared, confirming drug-induced etiology. While degenerati...

Persistent Drug-Induced Parkinsonism in Patients with Normal Dopamine Transporter Imaging

Functional neuroimaging for the dopamine transporter (DAT) is used to distinguish drug-induced parkinsonism (DIP) from subclinical Parkinson's disease (PD). Although DIP patients who show a normal DAT image are expected to recover completely, some do not. We investigated whether these patients showed changes in striatal DAT activity using semi-quantitative analysis of 18F-FP-CIT PET data. DIP p...

HIV-associated parkinsonism reversed with antiretroviral therapy

Human immunodeficiency virus (HIV) infection can cause variable movement disorders, including parkinsonism. HIV-related parkinsonism usually responds well to highly-active antiretroviral therapy (HAART), suggesting a possible reversible dysfunction of the dopaminergic system. We report the case of a 42-year-old man who presented with rapidly progressive symmetric parkinsonism, cognitive decline...
